Vermont Unveils Chronic Care Information System

Vermont last week unveiled its Chronic Care Information System, a statewide patient database and chronic disease management system that will let physicians track patients' care, view test results and exchange data, the Barre-Montpelier Times Argus reports.

The system will let providers track patient information, such as immunizations, said Vermont Commissioner Sharon Moffat. It also will identify gaps between need and availability on the regional level and help the state distribute vaccines to the providers who need them.

The system, which is expected to reduce errors and costs while improving patient care, will allow physicians to compare the type of treatment they provide with national standards of care and compare their patients' progress with other patients, the Times Argus reports.

Vermont Information Technology Leaders -- a not-for-profit organization created by the Vermont Legislature to develop the state's health information exchange and technology plan -- developed the information system. The group also developed a system that will be implemented in Rutland Regional Medical Center and Northeastern Vermont Regional Hospital by the end of February that lets emergency department physicians access with their consent patients' prescription records.

VITL subcontracted with GE Healthcare and Orion Health to provide data storage, IT architecture and software for the Chronic Care Information System. A pilot of the system is planned to begin by the end of June in the Mount Ascuntey area (Huff, Barre-Montpelier Times Argus, 1/14).
