Indiana Data Exchange Unveils Chronic Disease Management Tool

The Indiana Health Information Exchange has launched a chronic disease management program that aims to improve health outcomes, Healthcare IT News reports.

The program, called "Quality Health First," offers a multi-payer report with recent clinical and claims information to help doctors manage patients' health and care.

Among its features, the program:

  • Highlights patients who require follow-up care to manage chronic diseases, such as diabetes, asthma and high cholesterol;
  • Offers disease-based summaries based on the physician's patient population, as well as comparisons to the physician community as a whole; and
  • Provides doctors with reminders for patients who are due for preventive health screenings, such as colonoscopies and mammograms.

J. Marc Overhage, president and CEO of the Indiana Health Information Exchange, said the program aims to reduce health complications, improve adherence to evidence-based medical practices and increase patient satisfaction. He added that employers will benefit from a more productive and healthier work force, while payers will see a reduction in health care costs.

The Employers' Forum of Indiana created the Quality Health First program, which is run by IHIE.  The Fairbanks Foundation provided a $2 million grant, and Regenstrief Foundation provided a $1.7 million grant to help support the program's launch and ongoing development (Monegain, Healthcare IT News, 11/19).
